These terms explain how Preluma's programs work, what patients can expect, and how fees, cancellations, refunds, and discontinued services are handled.

Plain-language summary

You pay only for care that is delivered. If you paid in advance and leave a program, the value of undelivered services is refunded using the rates in your enrollment fee schedule. Your signed fee schedule and consent documents control if they differ from this page.

Purpose and scope

These Terms & Conditions apply to programs and associated services offered by Preluma Health Inc. ("Preluma," "we," "our," or "us"), including Surgical Prehabilitation, Fall Prevention, and Cancer Prehabilitation.

They apply whether services are purchased through a single payment, fixed-term package, or renewable block, and regardless of which Preluma practitioner delivers the service. They should be read with the consent form and individual fee schedule provided before services begin.

How the Preluma care model works

Preluma provides multidisciplinary preparation in the home. Programs are organized around a patient's health, goals, treatment timeline, capacity, and home environment.

Physiotherapist assessment

Every patient begins with an initial physiotherapist assessment. The physiotherapist screens for safety concerns, reviews relevant health information, and records appropriate baseline measures.

Kinesiologist-led training

A kinesiologist delivers the core in-home exercise program independently within the assessed plan and identified precautions. Behaviour-change support and encouragement are incorporated into these sessions.

Nutrition support

A registered dietitian provides nutrition consultation where included in the selected program or purchased as an additional service.

Additional support

Selected tiers may include physician health coaching. Registered psychotherapy may be offered as an optional add-on when screening or patient preference indicates that added support may be helpful.

Preluma practitioners may be independent contractors. Each regulated professional remains responsible for practising within their professional scope and standards.

Program structures

Surgical Prehabilitation

Surgical programs are fixed-term packages shaped by the number of weeks available before surgery and the selected intensity tier. Longer preparation windows may include more sessions. Higher tiers may add nutrition support and physician coaching.

Fall Prevention

Fall Prevention programs are purchased for a chosen number of weeks. The core program generally includes weekly kinesiology focused on strength, balance, mobility, and everyday function. Selected tiers may add nutrition or physician coaching.

Cancer Prehabilitation

Cancer Prehabilitation is offered in renewable two-week blocks because capacity can change during chemotherapy, radiation, and recovery. The initial block includes the physiotherapist assessment, nutrition consultation, and kinesiology sessions described in the patient's fee schedule. Renewal is always the patient's choice and is not assumed or pre-billed.

Optional add-ons

Psychotherapy and additional dietitian follow-ups are purchased and billed independently from the base program. Ending an add-on does not automatically end the base program, and ending the base program does not automatically cancel a separately scheduled add-on.

Safety screening, changes, and escalation

Every kinesiology session begins with a brief safety check for changes such as new pain, dizziness, falls, treatment effects, or other concerns. The practitioner may adjust, pause, or stop activity based on how the patient presents that day.

If an injury, new symptom, or other concern requires physiotherapist-led care, a physiotherapist may assume direct responsibility for future sessions. The revised clinical plan and any change in fees will be explained and quoted before the escalated service is delivered.

Preluma may pause or discontinue a service when it cannot be provided safely or appropriately. Preluma does not provide emergency medical care. Patients experiencing urgent or severe symptoms should contact emergency services or their responsible healthcare team.

Fees and payment

The individual fee schedule provided at enrollment lists the services, quantities, rates, and payment structure purchased by the patient. The fee schedule in effect at the time of purchase governs that purchase and any related refund.

Where a patient's individual fee schedule contains a different rate, including an individually quoted rate following clinical escalation, the individual fee schedule governs. Applicable taxes are shown on the patient's invoice.

Cancellation, withdrawal, and the core refund principle

Once a session or service unit has been delivered, its fee is earned in full and is non-refundable. If a patient paid in advance for services that have not been delivered when they cancel, withdraw, or discontinue, Preluma will refund the portion attributable to those undelivered services.

A patient who cancels or withdraws before a session or initial assessment is delivered owes nothing for that undelivered service and is entitled to a refund of any amount paid for it.

If HST was charged on the original payment, the refund includes the proportional HST attributable to the undelivered services.

How refunds apply by clinical stream

Surgical Prehabilitation and Fall Prevention

If a patient leaves before completing a fixed-term package, Preluma refunds the value of all undelivered services included in that package. Delivered services, including a completed initial physiotherapist assessment, remain non-refundable.

Cancer Prehabilitation

Each two-week block is invoiced and delivered independently. If a patient discontinues during an active block, Preluma refunds any undelivered services within that block. No payment or refund applies to a future block that the patient has not elected to renew.

Optional add-ons

Undelivered psychotherapy and additional dietitian services are refunded independently from the base program using the applicable per-service rate.

Escalated physiotherapist-led care

Services delivered before escalation remain non-refundable at their original rate. Services after escalation are billed at the individually quoted rate. Preluma will invoice or refund any difference between the remaining prepaid balance and the revised services.

How to request a refund

Patients may request a refund by emailing admin@preluma.health. Requests are processed within 14 days of receipt and returned to the original payment method wherever possible.

Preluma will proactively notify patients of remaining unused prepaid sessions and upcoming package or block expiry dates. This notice is a courtesy and does not change the refund rights described above.

Patient responsibilities

  • Provide accurate and reasonably complete information relevant to safe participation.
  • Tell the practitioner about new symptoms, falls, injuries, treatment effects, or health changes before activity begins.
  • Follow the safety instructions and limits provided by the responsible healthcare professionals.
  • Do not use the website or educational materials as a substitute for individualized medical advice.
  • Review the consent form, fee schedule, and these terms before becoming financially responsible for a service.

General terms

Preluma programs complement but do not replace the care provided by a surgeon, oncology team, primary-care clinician, or other responsible healthcare professional. Outcomes vary and cannot be guaranteed.

Website content is general information and may not reflect a patient's circumstances. Preluma may update these terms prospectively. Changes do not alter the rates or refund calculation for services already purchased unless the patient agrees in writing.

If any part of these terms is found unenforceable, the remaining provisions continue to apply. These terms are governed by the laws of Ontario and the applicable federal laws of Canada.
